Inclusion of a child's name would require Form 35 C, duly completed.  Form '35 C' could be obtained as a Word Document by clicking on the link (DOWNLOAD HERE)

Present Sri Lankan passports of both parents.

If the child was born in Sri Lanka, the Birth Certificate (original and photocopy) would be required.

If the child was born outside Sri Lanka, the Certificate of Registration of Birth outside Sri Lanka (original and photocopy) issued by the Sri Lanka Mission abroad and the Citizenship Certificate (original and photocopy) issued by the Secretary, Defence would be required.

Three certified passport size colour photographs and the present passport (in case of a minor, the passport of the parent in which the name of the minor is endorsed) would also be required.

A fee of $.11/= would be levied, payable by cash, bank cheque (Personal or Company cheque not accepted)  or money order to the "Consulate General of Sri Lanka".  Please note that this fee would not be applicable should the inclusion of the child's name be made when obtaining a new passport.
